چکیده

BACKGROUND Clinical guidelines from the National Institute for Health and Clinical Excellence recommend the use of an intramedullary nail for the treatment of subtrochanteric fractures.1 Reverse oblique fractures are associated with varus malunion,2 which defunctions the abductors by placing them at a mechanical disadvantage and increases the lever arm across the fracture. Most modern nails have been designed with a lateral bend to accommodate a trochanteric entry point. This simple technique helps to avoid varus malreduction without the need for excessive traction or open reduction.

Ten-year evaluation of cementless Buechel-Pappas meniscal bearing total ankle replacement.

A porous-coated, cementless, congruent-contact, three-piece, meniscal-bearing total ankle replacement was developed and used clinically over a 2- to 10-year period for patients with disabling ankle arthritis. Polished titanium-nitride ceramic-coated Ti6Al4V tibial and talar components with a deep-sulcus trochlear groove and two lateral fixation fins for the talar onlay component were used. The ...

Radiographical and Damage Mode Analyses of Retrieved Agility Total Ankle Arthroplasty Systems

INTRODUCTION: The AGILITY (DePuy, Warsaw, IN) total ankle arthroplasty (TAA) system was introduced in response to the high failure rates associated with the first generation of total ankle arthroplasty. First generation failures were generally attributed to increased loads across cement-bone interface, increased bone resection and poor surgical technique. Advancements in product design have in...

Accuracy assessment of measuring component position after total ankle arthroplasty using a conventional method

BACKGROUND This study was to assess the accuracy of measuring the tibial and talar components position and to investigate the outlier rate of each component and predisposing factors related to component malalignment after total ankle arthroplasty (TAA) using a conventional method. METHODS One hundred fifty consecutive primary total ankle arthroplasty were performed using the three-component H...

Finite element analysis of a total ankle replacement during the stance phase of gait.

Total ankle replacement (TAR) designs have still several important issues to be addressed before the treatment becomes fully acceptable clinically. Very little is known about the performance, in terms of the contact pressures and kinematics of TAR when subjected to daily activities such as level gait. For this purpose, an explicit finite element model of a novel 3-component TAR was developed, w...

The effect of accuracy of implantation on range of movement of the Scandinavian Total Ankle Replacement.

When performing the Scandinavian Total Ankle Replacement (STAR), the positioning of the talar component and the selection of mobile-bearing thickness are critical. A biomechanical experiment was undertaken to establish the effects of these variables on the range of movement (ROM) of the ankle. Six cadaver ankles containing a specially-modified STAR prosthesis were subjected to ROM determination...
